What Documents Do I Need to Submit?
The documents required depend entirely on whether you’re filing new claims, supplemental claims, or both. Below is a clear and detailed breakdown of what each type requires.
If You Are Filing New Claims
You will need to submit the following three documents:
- 526-EZ Form:
- The primary application for disability compensation.
- 526-SISOC (Statement in Support of Claim):
- Your personal written statement about your service-connected conditions.
- 526-Nexus Letter:
- An evidence-based nexus letter explaining how your condition is connected to your service.
If You Are Filing Supplemental Claims
Submit these three documents:
- 995 VA Form
- Used to request a review or provide new supporting evidence.
- 995-SISOC (Statement in Support of Claim)
- Updated statement supporting your supplemental request.
- 995-Nexus Letter
- Updated or new medical rationale connecting your condition to service.
Note: We use evidence-based nexus letters. An evidence-based nexus letter is a medical opinion that links a veteran’s current condition to their military service using objective evidence - such as medical records, service records, and medical research. It provides a clear, fact-supported explanation showing that the condition is at least as likely as not related to service.
If You Are Filing Both New and Supplemental Claims
You’ll submit all six documents:
- 526-EZ
- 526-SISOC
- 526-Nexus
- 995
- 995-SISOC
- 995-Nexus
This ensures every condition you are filing for, whether new or previously denied, has the required supporting documents.
